This gentleman was referred by his general dentist with a six-month history of this firm ulcerated slowly growing lesion on the left lateral border of his tongue. Full intraoral and head and neck evaluation yielded was otherwise negative. After reviewing his medical and social history which was negative for tobacco and alcohol, the first thing I looked for, in order to rule out chronic irritation, was a sharp tooth or dental restoration in the area. There was none. No lymphadenopathy was present, and a decision was made to excise and biopsy the lesion. Understanding that the differential diagnosis included SCCa but was not definitively such, I excused the lesion WITHOUT real concern for margins. ⁣⁣ ⁣⁣ After testing, found that this was NOT squamous cell carcinoma which is the type of cancerous lesion one might have in the differential diagnosis. It was a non-specific ulceration, likely secondary to chronic irritation or habitual and repetitive tongue biting.
