Dr. Rittenberg is the division head for Oral and Maxillofacial surgery at Mount Sinai Hospital, part of Sinai Health System and the former president of the Ontario Society of Oral Maxillofacial Surgeons.

A Toronto native, Dr. Brian Rittenberg graduated from the University of Western Ontario in 1992 with a Bachelor Degree in Psychology. He then completed his Dental Degree, specialized training in Oral and Maxillofacial Surgery and a Masters in Bone Biology at the University of Toronto and Toronto General Hospital in 2003.

Since starting his practice, Dr. Rittenberg enjoys teaching dental interns and residents at Mount Sinai Hospital, where he has also cultivated an interest in the surgical management of Temporomandibular Joint (TMJ) Disorders. He is also an attending surgeon in the Regional Treatment Centre for Temporomandibular Joint Reconstruction there.

In addition to having a special interest in the surgical management of TMJ disorders, Brian enjoys practicing all facets of Oral and Maxillofacial Surgery, including the removal of wisdom teeth, the placement of dental implants, the evaluation and surgical treatment of pathology (diseases of the mouth) and corrective jaw surgery.

Dr. Rittenberg has also served as a line officer and ultimately president of the Ontario Society of Oral and Maxillofacial Surgeons, is involved with the Royal College of Dentists of Canada (the national licensing body for all dental specialists) and is a fellow of the ITI – International Team for Implantology.

How to be more efficient with bulkfill composites

For July’s tip of the month, Dr. Arthur Volker has a conversation with Cosmedent’s Bennett Cochran (@bennettskis2013 on Instagram) about how he uses opaquers to make his cervical restorations invisible. 

“If the patient is periodontally sound, often we can use direct composite as a predictable means to address cervical lesions,” says Dr. Volker as he shares the immediate post-op results of a Class V case that has obvious transition zones. 

To address the transition zone, Dr. Volker recommends avoiding the use of a translucent composite because it will become very visible and may even have a grey appearance. 

Instead, one of the best ways to add opacity to the cervical region and help mask the transition line is to use flowable, liquid opaquers like Cosmedent’s Creative Color line. 

Dr. Volker then guides you through the simple steps he takes when using opaquers on cervical restorations to achieve ideal esthetic results.

1. Place your dentin layer composite
2. Add your opaquer to a Cosmedent Brush #1
3. Place your Creative Color Opaquer over the transition line and cervical area (this will also help subsequent layers of composite to adhere)
4. Make sure you have sufficiently opaqued the area before placing your Renamel Microfill composite

Dr. Volker is a Diplomate of the World Congress of Minimally Invasive Dentistry and is a Fellow of the Academy of General Dentistry and the American College of Dentists. He was a clinical attending at the Coler-Goldwater Specialty Hospital and Nursing Facility on Roosevelt Island, and is in private practice in New York, with an emphasis on cosmetic and minimally invasive dentistry. Dr. Volker has published articles and lectures nationally and internationally on such topics as cosmetic dentistry, minimally invasive dentistry, dental materials and dental implants.

Finishing and Polishing Composite Restorations

Composite resins have led to significant advancements in restorative dentistry offering patients a highly aesthetic and minimally invasive option. Ii is an extensive topic and also quite important from examination point of view. In the first part we discussed the composition and classification of composite and in this part we will look at the clinical techniques for anterior composite restorations. 


Composite Resins: Clinical Technique- Anterior Composites

The hypersensitive dentin can be treated by 2 main mechanisms: first type is the one we just saw i.e. Occluding the open dentinal tubules  and the second being Depolarization of the nerve fibres. The first group of products works by occluding the open tubules and decreasing pulpal fluid flow. This group includes: fluorides, fluoride varnishes, Fluoride iontophoresis, tissue fixatives, oxalates, remineralising agents, and Pro-Argin technology. The second group of products works by depolarizing the nerve so that it cannot transmit the pain response. The ingredient that produces this effect is potassium nitrate used in a concentration of 5%. Some newer agents used to aaleviate dental hypersensitivity are Laser, Pre-hybridized dentin and Bioglass (Novamin).


Dentin Hypersensitivity- Treatment modalities

Dentin hypersensitivity is defined as pain derived from exposed dentin in response to chemical, thermal, tactile or osmotic stimuli which cannot be explained as arising from any other dental defect or disease. 14.3% of all dental patients, between 8% and 57% of adult dentate population, and up to 30% of adults at some time during their lifetime experience dentin hypersenitivity.  Dentinal hypersensitivity has been shown to peak in 30 to 40 year olds and then rise again when in their 50s. Various clinical conditions thought to play a role in the development of DHS include enamel attrition and erosion, corrosion, abrasion and abfraction. Periodontal tissue loss or gingival recession is another major predisposing factor since this leads to exposure of cervical and root dentin. A variety of theories have been suggested to help explain the mechanism involved in the etiology of dentinal hypersensitivity. The transducer theory, the modulation theory, the "gate" control and vibration theory, and the hydrodynamic theory have all been presented and discussed throughout the years. None of these mechanisms full explain dentin hypersensitivity, indicating unexplained mechanisms are possibly responsible.

Dentin Hypersensitivity: Etiology & Pathogenesis

Pulp testing involves attempting to make a determination of
the responsiveness of pulpal sensory neurons. Laser Doppler Flowmetry (LDF) is a non-invasive method of assessing and measuring the blood flow of pulp tissue. It is based on the principle that the light beam’s frequency will shift when hitting moving red blood cells but will remain unshifted as it passes through static tissue. Pulse Oximetry is a non-invasive method to measure the oxygen concentration in the blood & the pulse rate, in which the  light absorbed depends on the ratio of oxygenated and deoxygenated hemoglobin in the blood. Percussion & Bite test include the use of Tooth Slooth and FracFinder. Test cavity is an invasive, irreversible test which 
can be used in case of full coverage crown. Selective anesthesia involves periodontal ligament (intraligamentary) injection beginning from distal sulcus of the  posterior most tooth. 
Pulp vitality tests: Thermal (Cold & Heat), Electric tests, Pulpal diagnoses:

Pulp testing involves attempting to make a determination of
the responsiveness of pulpal sensory neurons. The tests
involve thermal or electrical stimulation of a tooth in order to
obtain a subjective response from the patient, or the tests may
involve a more objective approach using devices that detect the
integrity of the pulpal vasculature. The sensitivity, which is the ability of a test to identify teeth that are diseased, is estimated to be 0.83 for the cold test, 0.86 for heat test and 0.72 for the electric test. Specificity relates to the ability of a test to identify teeth without disease. From the results of the testing, it was found that the cold test had an accuracy of 86%, the electric pulp test 81%, and the heat test 71%.

A Ferrule is a 360 degree collar of the crown surrounding the parallel walls of the dentin extending coronal to the shoulder of the preparation. When properly placed, it reduces the incidence of fracture in endodontically treated teeth. Ferrule combines a shoulder preparation with parallel coronal dentin walls and improves overall resistance of restoration. Ferrule design incorporates nearly parallel walls, a minimum height of 2mm, width of at least 1mm and preferably placed circumferential. Another important aspect to be considered while placing a ferrule is the biologic width. In severely damaged tooth with no clinical crown available, surgical crown lengthening or orthodontic  extrusion can be considered.

Internal derangement of TMJ is a disruption of the internal aspects of the TMJ, in which an abnormal relationship exists between the disc and the condyle, glenoid fossa and articular eminence. Trauma is the most common cause. Types of trauma can be macrotrauma, microtrauma and indirect trauma. Management of ID of TMJ involve conservative measures for stages I-III and surgical intervention for the latter stages. Medication and physiotherapy are quite helpful in relieve pain and increasing the range of motion of the joint. Other modes of management are Arthrocentesis, Arthroscopy, Disecectomy, Eminectomy, Condylar shave and ultimately total joint replacement.

Internal derangement of TMJ is a disruption of the internal aspects of the TMJ, in which an abnormal relationship exists between the disc and the condyle, glenoid fossa and articular eminence. Disc displacement can be subdivided as with and without reduction. Anteromedial displacement of disc is more common. A better understanding of the TMJ anatomy and functions aids in understanding the pathophysiology of disc derangement better. Wilkes classification describes the various stages of disc derangement and associated symptoms clinically and radiologically.

Recording accurate jaw relations is of paramount importance when preparing a prosthesis. Vertical dimension is based on the principle that the repetitive contracted length of the elevator muscles determines the VD at occlusion. The VDO refers to the vertical position of the mandible in relation to the maxilla when the upper and lower teeth are intercuspated at the most closed position. Maximal intercuspation of the posterior teeth determines the existing VDO. If the VDO must be changes, it should be determines at the point of anterior teeth contact. VDR or physiologic rest position of the mandible is the position when the elevator and depressor muscles are in a state of equilibrium or balance. this position most commonly results in a separation of the maxillary and mandibular teeth of about 3mm at the first premolar region. This separation is called the "interocclusal space".
